What could cause raw skin right below the head of the penis? -

My husband and I had sex and I noticed that an area right below the head of his penis was raw looking and red. Looked like it was kind of bleeding. What would cause this? Is this a sign that the sex was too rough? We are monogamous, so that cheating is not an issue. Any insight would be helpful.

Maybe too rough or just a lack of lubrication during masturbation.
