It should be taken from the base of your penis (the side you see as you look down) to the tip of the glans.
get yourself a Bob the Builder tape measure and see how that works out for you
ok, I m done laughing now. get yourself hard, place the ruler at the base of your penis (right on the pubic bone) press yourself flat against the ruler and mark where the tip is.
